Periodontology is the dental specialty that deals with diseases and treatment of the gums.

A healthy gum is one of the important factors that determine our overall well-being. The dental plaque that accumulates on the teeth, and the calculus that forms from it, can cause the gums to become inflamed: they may swell, turn red, and the patient may complain of gum bleeding or, in more severe cases, tooth loosening. With proper and thorough, regular oral hygiene treatment, the complete healing of the gums can be achieved at this stage, which is why we place special emphasis on oral hygiene treatments.

With a six-month regularity, and prior to any dental intervention, we recommend the so-called Guided Biofilm Therapy to our patients, which involves the removal of biofilm, plaque, calculus, and stains that form on the teeth.

The long-term success of our oral surgery and aesthetic dental treatments is also due to the fact that patients undergo a strict and thorough (but pain-free) periodontal pre- and post-treatment. This minimizes the number of bacteria causing gum disease, thereby increasing the lifespan of natural teeth and implanted prosthetics. Regular oral hygiene treatment is a condition for maintaining the long-term health of the gums.

If gum inflammation persists for a longer period, the inflammation spreads to the deeper tissues of the gums, the alveolar bone, and the periodontal ligament. The supporting structure of the tooth begins to decay, a pocket forms around the tooth, and eventually, the tooth becomes loose.

Treatment of periodontal disease

Diagnosis

We have many years of experience in treating established periodontal diseases and apply well-established therapeutic protocols. During the planning of necessary treatments, we perform a thorough clinical examination:

  • Examination of oral hygiene
  • Examination of the gum and periodontal tissues using oral, radiological, immunohistochemical, and bacteriological methods
  • Assessment of periodontal pockets, periodontal status assessment
  • Determining tooth mobility
  • Assessment of genetic and other predisposing factors


Based on the above information, we determine the stage of the disease and then develop an individual treatment plan for the resolution of the periodontal disease.

Oral hygiene phase, patient preparation and motivation
The dental hygiene treatment phase begins with multiple sessions of calculus removal/Biofilm therapy, during which we teach the patient the correct brushing technique and the use of special oral care tools, continuously monitoring the development of the patient’s independent oral care. This phase can last for several weeks.

What is GBT?
GBT stands for Guided Biofilm Therapy, which means directed biofilm therapy.
What is biofilm?
It is a collection of microorganisms that have adhered to a certain surface, forming a continuous coating on teeth, dental restorations, fillings, gums, and implants.

Why GBT?

On the teeth, biofilm forms where bacteria, plaque, and later calculus accumulate. When we remove the biofilm itself, calculus formation is expected to occur much later, thus creating a long-lasting feeling of cleanliness, aesthetic appeal, comfort, and an inflammation-free state.

What do you need to know about the GBT system?
It is the most modern Swiss method, gentle on both the teeth and gums. This special procedure contributes to maintaining a healthy oral cavity and prevents numerous oral and/or dental diseases.
What is calculus?
Calculus is calcified dental plaque, which forms on the biofilm that was not removed. Traditional home oral hygiene routines cannot remove calculus; it requires a professional. Calculus is home to numerous bacteria that cause inflammation of the gums, and later, responsible for the breakdown of the supporting structures of the gums and implants. It also plays a role in other areas of the body, as confirmed by the bloodstream.
How is GBT different from traditional calculus removal?
  • It is gentler on the tooth enamel

  • It is safe to use on implants and dental restorations

  • It helps establish and maintain proper oral hygiene

  • It is painless and does not cause sensitivity

  • It can also be used in patients with periodontal disease

If your gums and periodontium are healthy, or if after a treatment series we have successfully managed to restore the condition of the gums, then the treatments mentioned above must be repeated at appropriate intervals as maintenance. In this case, there are no further steps to take. However, if we are dealing with periodontal disease, the following steps may become necessary after the hygiene procedures:

Closed Curettage
For not too deep (4-6 mm) periodontal pockets, the so-called closed curettage works well, which is performed under local anesthesia. During the cleaning treatment below the gum, we use special instruments to remove the infected root cement layer and the inflamed pocket wall. After a few weeks of healing, a follow-up examination is performed, during which we check compliance with the prescribed home oral hygiene routine, its effectiveness, and the results of previous treatments. If necessary, further treatments are recommended.
Open Curettage, Tooth Retention Regenerative Minor Surgeries

The treatment for pockets that were not treated with closed curettage is performed with open curettage, which involves lifting the gum and allows for perfect inflammation-free healing under visual inspection.

The goal of regenerative minor surgeries is to promote the regeneration of bone tissue next to the teeth using special bone grafting materials (Bioss® (Geistlich), own bone, and various collagen membranes. Additionally, we successfully use enamel matrix protein preparation: Emdogain® (Straumann) during the surgeries.

Plastic surgery of the gums
A healthy gum is not only the foundation for healthy and functional teeth, but also a key element in the healthy oral environment and an attractive, aesthetic smile. We recommend the use of mucogingival/gum plastic surgery procedures to address aesthetic or functional problems in the following cases:
  • Single or multiple gum recessions, potentially causing tooth root sensitivity

  • Missing firm gum tissue around teeth or implants

  • Excessively attached vestibule

  • Improper attachment of the gum papilla

  • Other aesthetic problems caused by mucogingival lesions

Personal Oral Hygiene Program

The key to our long-term success is ensuring that the patient can maintain healthy oral hygiene even after completing a series of dental treatments.

For this purpose, within the framework of an individual oral hygiene program, we teach our patients the appropriate brushing technique for their specific cases and, where necessary, the use of professional oral hygiene tools at home. Our dental hygienists help select the appropriate electric toothbrush, dental floss, interdental/soft-bristle toothbrush, or mouthwash tailored to your specific needs.

Based on the condition of the gums, we determine how often you need to visit our clinic for oral hygiene treatments.
